FreshPath Ministry Inc. is non-profit organization that has been in operation since April of 2002. We were established as a 501c (3) June of 2003.
FreshPath Ministry is performing with a strong Board of Directors, management team, staff, independent contractors and a large volunteer staff.
FreshPath Ministry currently operates as a state provider for skilled home health care services, housing services and referrals, case management and substance abuse recovery services to New Mexico residents. We also manage a transitional semi-independent living facility in Albuquerque, NM.
We have evolved into a one stop program that assists individual with maintaining the basic stability required for social success and well being.
FreshPath Ministry is currently licensed through the Department of Health to provide skilled nursing services to the residents of New Mexico. Under the New Mexico State Long Term Services, we are providers for the Disable and Elderly Waiver Program. We bill Medicaid, commercial and private pay for home health care services.
In addition, FreshPath Ministry is a Health Care Recovery provider. We act as the referral case management agency for housing opportunities through Help- NM, St. Martins and Steel Bridge.
FreshPath Ministry Inc. (FPM) is a non-profit 501c 3 status organization that provide community based services and home healthcare services to individuals having social, mental and physical disabilities to meet their day-to-day objectives.
FPM’s team of case managers assist individuals in community support recovery services to improve their quality of life by providing navigating assistance and referral resources for housing, education, community interaction services, life skills and home management. We are a charitable group that has pride in helping our fellow mankind in times of need. Our sustainability is mainly obtained by generous contributions and donations in conjunction with the collaborating relationships we have formed with many other state, city, local organizations, and businesses that share our vision of creating a safe and welcoming environment for our clients. Thus, we rely on the networking model to continue to provide our clients with the best services possible. Our broad networking frame includes HUD housing, Help NM, NM State Office of African American Affairs, local Churches and other human resource service providers among others.
FPM is also a licensed home health care agency for the state of New Mexico, and a licensed providers for Adult Long Term Services’ Disable and Elderly Centennial Care federal funded services. FPM is in good standards with all aspects of requirements put forth by ACS, DOH, Human Services and Centennial Care Networks. Although our number of consumers are constantly changing depending on the services provided, FPM scope of service at this time is the Albuquerque Metro area and surrounding counties, including but not limited to, Bernalillo, Sandoval, Valencia and Santa Fe counties.
